Sacubitril, valsartan

Indications/Uses

Reduce the risk of CV death & hospitalization for heart failure in patients w/ chronic heart failure (NYHA class II-IV) & reduced ejection fraction. In conjunction w/ other heart failure therapies, in place of ACE inhibitor or other ARB. Essential HTN.

Dosage/Direction for Use

Heart failure Target dose: 200 mg bd. Starting dose: 100 mg bd. Double dose every 2-4 wk to the target dose of 200 mg bd as tolerated by the patient. Patient not currently taking ACE inhibitor or ARB & those previously taking low doses of these agents Starting dose: 50 mg bd. Severe renal (eGFR <30 mL/min/1.73 m²) & moderate hepatic (Child-Pugh B) impairment Starting dose: 50 mg bd. Essential HTN Starting dose: 200 mg once daily. Patient whose BP could not be adequately controlled w/ Meravo 200 mg once daily May be increased to 400 mg once daily. Moderate hepatic impairment (Child-Pugh B) Starting dose: 100 mg bd.

Administration

May be taken with or without food.

Contraindications

Hypersensitivity. Known history of angioedema related to previous ACE inhibitor or ARB therapy. Hereditary angioedema. Concomitant use w/ ACE inhibitors; not to be administered until 36 hr after discontinuing ACE inhibitor therapy. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (eGFR <60 mL/min/1.73 m²). Severe renal impairment w/ eGFR <10 mL/min/1.73 m² & patients undergoing dialysis. Pregnancy.
