Saxenda

Adjunct to a reduced-calorie diet & increased physical activity for wt management in adults w/ an initial BMI of ≥30 kg/m² (obese), or ≥27 to <30 kg/m² (overwt) in the presence of at least 1 wt-related comorbidity eg, dysglycaemia (pre-diabetes or type 2 DM), HTN, dyslipidaemia or obstructive sleep apnoea. Adjunct to a healthy nutrition & increased physical activity for wt management in adolescents ≥12 yr w/ obesity (BMI corresponding to ≥30 kg/m² for adults by international cut-off points), & body wt >60 kg.

SC Initially 0.6 mg once daily. Increase to 3 mg once daily in increments of 0.6 mg w/ at least 1-wk intervals.

May be taken with or without food.

Hypersensitivity.

Must not be administered intravenously or intramuscularly. Risk of acute pancreatitis; cholelithiasis & cholecystitis; increased heart rate; dehydration. Increased risk of hypoglycaemia in patients w/ type 2 DM receiving concomitant insulin &/or sulfonylurea. Should not be used w/ another GLP-1 receptor agonist in patients w/ type 2 DM. Must not be used as substitute for insulin in patients w/ DM. Not recommended in patients w/ CHF NYHA class IV; patients treated w/ other products for wt management; patients w/ obesity secondary to endocrinological or eating disorders or to treatment w/ medicinal products that may cause wt gain; patients w/ inflammatory bowel disease & diabetic gastroparesis; patients w/ severe renal impairment (CrCl <30 mL/min) including ESRD; patients w/ severe hepatic impairment; elderly ≥75 yr. Caution in patients w/ thyroid disease; patients w/ mild or moderate hepatic impairment. Caution w/ driving or using machines if dizziness occurs. Should not be used during pregnancy & breast-feeding. Safety & efficacy in childn <12 yr have not been established. Episodes of clinically significant hypoglycaemia have been reported in adolescents ≥12 yr.

Headache; nausea, vomiting, diarrhoea, constipation. Hypoglycaemia; insomnia; dizziness, dysgeusia; dry mouth, dyspepsia, gastritis, GERD, upper abdominal pain, flatulence, eructation, abdominal distension; cholelithiasis; rash; inj site reactions, asthenia, fatigue; increased lipase & amylase.

Clinically relevant interaction w/ active substances w/ poor solubility or narrow therapeutic index eg, warfarin cannot be excluded.
